Poor topology causes odd deformation—like lids not closing smoothly—when animated. Wrong proportions break realism (human eyes show ~1/3 of the iris, so too big/small looks off). Flat shading fails to mimic the eye’s reflective, moist surface.

Fix it by overlaying your model on a real eye reference for proportions, adjusting edge loops to follow natural curves (for topology), and tweaking materials: high specular for shine, low roughness for a wet look.
